End of Life Care

Are you living with a long term illness?
Are you looking after someone with a long term illness?
Have you recently been touched by the death of a loved one, or know someone who has?

At some point in our lives most of us will or have come across a situation where we or someone we love has been diagnosed with a serious illness, disability or terminal illness.

If it happened to you have you ever thought about where you would like to be cared for, what treatments you would and wouldn’t want, who would look after you or speak on your behalf if you weren’t able to?

These are difficult questions however it is important to consider them in advance to help family, friends and carers.

The video to the right of this page is of Dr Emma Whitehouse, lead for End of Life Care for Surrey Heath CCG, talking about End of Life Care.

Useful resources

The NHS website has a useful booklet  ‘Planning for your future: A guide’ – this gives you information and background on the questions posed above and the options that you may wish to consider.

More information is available here.

Advance care plan
Another useful booklet is for you to record and keep is the Advance Care Plan.  It is not a legally binding document but more of a place to help guide you during your illness.

If you have any questions about these please speak to your GP or another health care professional involved in your care.
